Testosterone therapy carries a well-replicated evidence base for one specific problem, a much thinner one for two others, and an unsettled debate around the delivery method a lozenge represents. Anyone weighing a testosterone troche for females deserves all three of those layers, not the flattering slice.

What follows rounds up where the science currently sits on desire, energy, and mood, along with what the formulation argument means in practice. None of this is medical advice, and none of it replaces lab work and a conversation with a clinician who knows your history.

What a Troche Is and How It Delivers Testosterone

A troche is a small lozenge, pronounced tro-key, that dissolves between the cheek and gum or under the tongue over two or three minutes. A compounding pharmacy presses testosterone into a flavored base, usually gelatin or polyethylene glycol, at whatever strength a prescription specifies. Flavoring matters more than it sounds, since testosterone tastes bitter enough that women skip doses without it.

Clinics reach for troches for practical reasons. There is no needle, no daily cream to rub in and keep off a partner’s skin, no implanted pellet that cannot be removed if it disagrees with you. Strengths can be dialed to a specific milligram figure rather than accepting whatever a commercial product offers, which matters because commercial testosterone products sold in the United States are formulated for male dosing.

The mechanical claim behind the format is that testosterone crosses the lining of the mouth straight into the bloodstream, sidestepping the digestive tract and the liver’s first pass. That claim is where the interesting argument starts, and it gets its own section further down.

How Buccal Absorption Differs From Creams, Patches, and Pellets

Each delivery route puts testosterone into the blood on a different schedule, and the schedule shapes both the effect and the risk. Transdermal creams, gels, and patches release slowly through the skin across the day, which keeps levels relatively flat. That steadiness is one reason the majority of the randomized trials behind current guidance used transdermal preparations.

A troche behaves differently. Absorption through the oral lining is fast, and clearance is fast too, which is why prescriptions frequently call for more than one dose a day. More doses mean more chances to forget one, and inconsistent dosing muddies any attempt to judge whether the therapy is working.

Pellets sit at the opposite end. Implanted under the skin, they release over months and cannot be dialed back if levels climb too high. Injections produce peaks and troughs that can overshoot the premenopausal range. The 2019 international consensus statement on testosterone therapy for women drew a line around all of this, stating that its recommendations do not extend to injectables, pellets, compounded preparations, or any formulation producing blood concentrations above physiological levels.

Why Women Produce Testosterone in the First Place

The ovaries and adrenal glands produce testosterone throughout adult life, and the body puts it to work in two ways. Some circulates and binds directly to androgen receptors found in the brain, bone, muscle, breast, and blood vessel tissue. The rest serves as raw material, converting into estradiol through an enzyme called aromatase. Changing testosterone therefore nudges estrogen supply as well, which is part of why the two hormones are difficult to study in isolation.

That dual role explains the breadth of claims made about testosterone in women’s health. Receptors in that many tissues make it plausible that the hormone touches energy, thinking, bone density, and body composition. Plausible is not the same as demonstrated, and the gap between the two is where most of the marketing in this category lives. Biological plausibility generates hypotheses. Randomized trials settle them.

How Testosterone Levels Change Through Perimenopause and Menopause

Testosterone does not crash at menopause the way estrogen does. It declines gradually from a woman’s twenties onward, so a woman reaching fifty may already be producing about half what she did at twenty-five, with no single event marking the drop. Estrogen falls sharply across the menopause transition by comparison. The two hormones follow different curves, which is why symptoms that arrive suddenly are more often an estrogen story.

Surgical menopause changes the arithmetic. Removing both ovaries eliminates a major production site overnight, and the resulting androgen drop is abrupt rather than gradual. Women in that group are the most consistently studied population in testosterone research.

One finding complicates all of it. The international consensus statement concluded that no cut-off blood level for any measured circulating androgen separates women with sexual dysfunction from women without it. A number on a lab report does not, by itself, identify testosterone as the source of a symptom.

What the Research Shows About Libido and Sexual Desire

Sexual desire is the one outcome where the evidence is solid. A systematic review and meta-analysis published in The Lancet Diabetes and Endocrinology pooled 36 randomized controlled trials covering 8,480 women and found improvements in desire, arousal, orgasm, and sexual satisfaction. That analysis became the foundation for the clinical practice guideline recommending testosterone for postmenopausal women with hypoactive sexual desire disorder.

The size of the effect is worth stating plainly, because marketing copy tends to inflate it. Working from those pooled trials, the 2019 global consensus statement reported that testosterone at doses approximating premenopausal physiological concentrations produced an average of about one additional satisfying sexual event per month above placebo, alongside gains in desire, arousal, orgasmic function, pleasure, and responsiveness, plus a reduction in sexual distress. Real, measurable, and modest. Eleven medical societies endorsed the statement, among them the Endocrine Society, the International Menopause Society, and the International Society for the Study of Women’s Sexual Health.

The scope of that recommendation is narrow on purpose. Because the pooled trials recruited women diagnosed with hypoactive sexual desire disorder or generalized female sexual dysfunction, the panel stated the findings cannot be generalized to other subtypes or to women without sexual dysfunction. If women’s sexual health and libido are the primary concern, this is the body of research that applies.

Understanding Hypoactive Sexual Desire Disorder

Hypoactive sexual desire disorder is a formal clinical diagnosis, not a synonym for having less interest in sex than you did at thirty. It requires persistently absent or reduced sexual desire that causes the woman personal distress. Both halves matter. A woman whose desire has dropped but who is untroubled by the change does not meet criteria, and treating her would mean treating a number rather than a person.

Reaching that diagnosis properly means ruling things out first. The Lancet meta-analysis found insufficient randomized evidence to back testosterone for any menopausal symptom other than low sexual desire, and its authors wrote that effects on individual wellbeing, along with musculoskeletal and cognitive health and long-term safety, still warrant further investigation.

That phrasing deserves care, because unproven and disproven are different states. Nearly all of the trials in question were designed with sexual function as the primary outcome. Wellbeing and energy were secondary measures, assessed with varying instruments in study populations selected for a different complaint. A trial built to answer one question answers others poorly. The honest position is that the evidence base has not tested this properly yet, not that testosterone has been shown to do nothing for energy.

There is also a practical problem with treating fatigue as a hormone question first. Persistent fatigue during menopause commonly traces back to fragmented sleep from night sweats, iron deficiency, thyroid dysfunction, undiagnosed sleep apnea, or depression. Each of those has its own workup and its own treatment. Reaching for testosterone before ruling them out risks leaving a treatable condition in place.

Where Mood Research Currently Stands

Pooled trial data has not shown testosterone to improve mood or thinking in menopausal women. The Lancet analysis reported that finding, and a later systematic review published in 2022 reported the same one. For depressive symptoms and for cognitive complaints, the aggregate signal has been absent.

The samples behind those findings are thin, and thin evidence cuts both ways. Pooled cognitive outcomes drew on three randomized trials totaling 213 women, of whom 102 received testosterone. Depressive symptoms drew on four trials totaling 636 women, 313 of them treated. Those are small numbers for questions this consequential; the trials varied considerably in design and measurement, and none set out to study mood as the primary endpoint. Researchers working in this area have been explicit that the question remains open rather than settled.

Why Women Often Report Feeling Better Anyway

Women who start testosterone frequently report more energy and steadier moods, and dismissing those reports would be both rude and unscientific. Several explanations fit the pattern without requiring testosterone to act directly on mood.

Most women prescribed testosterone are already taking estrogen therapy, which independently reduces night sweats and improves sleep, and sleeping through the night reliably lifts both energy and outlook. Improvement in a sexual relationship changes how a woman feels day to day. Placebo groups in these trials improve as well, which is precisely why effects get reported as gains above placebo rather than as raw before-and-after numbers. And a woman who has finally found a clinician who takes her symptoms seriously, orders proper labs, and follows up every few months is receiving something therapeutic that has nothing to do with the molecule.

None of this means a woman is imagining her improvement. It means that attributing that improvement to one hormone, when four or five variables changed at once, is guesswork dressed up as causation. Women tracking mood changes and irritability through the menopause transition are usually dealing with several moving parts at the same time.

The Formulation Question Researchers Keep Raising

Here is the part most articles on this topic leave out. Prescribing guidelines for testosterone in women state that compounded buccal lozenges and troches result in substantial gut absorption, because saliva carrying dissolved hormone gets swallowed while the lozenge sits in the mouth. On that basis, the guidance directs clinicians to treat troches as equivalent to oral testosterone rather than as a mucosal delivery method.

That reclassification matters, because oral testosterone raises LDL cholesterol, lowers HDL cholesterol, and is specifically not recommended for hypoactive sexual desire disorder under current guidance. The route that sounds gentlest may be the one that behaves most like a pill.

Compounding pharmacies push back, pointing to rapid absorption across the oral lining and the lower doses troches require compared with capsules. The disagreement is quantitative rather than philosophical. Nobody disputes that some testosterone crosses the mucosa and some gets swallowed. What remains unsettled is the ratio, and the ratio determines which safety profile applies. Batch-to-batch potency variation adds a second layer of uncertainty, since compounded preparations are not subject to the same manufacturing verification as commercially produced drugs. A woman considering a testosterone troche for females should know that the largest consensus statement in this field explicitly excluded compounded preparations from its recommendations.

Why No Testosterone Product Is FDA Approved for Women

No testosterone product carries FDA approval for use in women in the United States, for any indication. Products designed specifically for women have been developed and submitted for review over the years, and none currently holds American approval. The consensus statement addressed the gap directly, advising that in the absence of an approved female product, male formulations may be used judiciously at female doses with blood concentrations monitored to confirm they stay within the physiological range.

That leaves clinicians two off-label paths. They can prescribe a male-labeled product at a fraction of the male dose, or they can order a compounded preparation such as a cream or a troche. Off-label prescribing is legal, common, and often appropriate across medicine. What it does not provide is an FDA-reviewed dosing schedule, an approved safety label for this population, or the post-market surveillance that follows an approved drug.

Compounded products sit further out still. They are not reviewed by the FDA for safety or efficacy at all, and a testosterone troche for females falls squarely in that category. Knowing that going in is different from finding it out later.

Known Side Effects and What Monitoring Involves

Acne and unwanted hair growth are the side effects reported most consistently across trials, and both track with dose. At physiological doses, meaning levels held within the range of a healthy premenopausal woman, they are usually mild and reversible. At supraphysiological levels, the androgenic effects broaden, and some of them, including voice deepening, may not reverse when treatment stops. That is the central argument for keeping levels measured rather than assumed.

Monitoring in the guidelines is specific. A three to six-month trial is suggested at a dose producing a mid-to-high normal premenopausal blood level in a reference assay, for postmenopausal women with a properly diagnosed hypoactive sexual desire disorder and no contraindications. If a woman has not responded after six months, the guidance says to stop. That stopping rule is one of the more useful things in the document, because it protects women from indefinite therapy that is not working.

Any responsible bioidentical hormone therapy program builds this in from the start, with baseline labs, follow-up testing to confirm levels have not drifted above range, and a scheduled decision point rather than an open-ended prescription.

What Blood Testing Can and Cannot Tell You

A testosterone level cannot diagnose the cause of a symptom. The international consensus was unusually blunt here, concluding that no cut-off blood level for any measured circulating androgen distinguishes women with sexual dysfunction from those without it. A woman with a low reading may feel fine. A woman with a mid-range reading may have marked symptoms. Treating the number rather than the woman gets this backward.

There is a technical wrinkle underneath as well. Female testosterone concentrations sit at the low end of what many standard immunoassays measure reliably, and results can vary between assay platforms. Reference laboratories using mass spectrometry produce more dependable numbers at these concentrations, which is why the guidance specifies a reference assay rather than any available test.

Testing still earns its place. It confirms that therapy has not pushed levels above the premenopausal range, tracks lipids and other safety markers over time, and rules out thyroid disease, anemia, and other conditions that produce overlapping symptoms.

Questions Worth Bringing to a Hormone Specialist

The most useful questions are narrow and specific. Ask which symptom is being treated and how improvement will be measured, because a plan aimed at low desire looks different from one aimed at fatigue, and only one of those has a supporting evidence base. Ask which formulation is being prescribed and why that one rather than a transdermal cream, then listen for whether the answer addresses absorption and monitoring or simply describes convenience.

Ask what the target blood level is, which assay the laboratory uses, and how often levels will be rechecked. Ask how long the trial will run before you and the clinician decide together whether it is working, and what happens if it is not. Ask what else has been ruled out, since fatigue and low mood carry longer differential diagnoses than most people expect.
